"font-size:14pt;font-family:宋体;"> "font-size:14pt;font-family:宋体;">为贯彻《**职业病防治法》，规范职业病危害评价的收费行为，维护委托方和评价机构的合法权益，促进职业病危害评价的工作更好的为企业服务，现就建设项目职业病危害评价收费的有关问题通知如下：
"font-size:14pt;font-family:宋体;"> "font-size:14pt;font-family:宋体;">一、职业病危害评价是建设项目前期工作中的重要环节，此项工作必须由依法设立的取得省级以上人民**卫生行政部门资质认证的职业卫生技术服务机构按照**《职业病危害因素分类目录》和《建设项目职业病危害评价规范》等文件的规定进行。
"font-size:14pt;font-family:宋体;"> "font-size:14pt;font-family:宋体;">二、职业病危害评价属于服务性收费，应当遵循公开、平等、自愿、有偿的原则。委托方有权自主选择有资质的职业卫生技术服务机构开展职业病危害评价工作。
"font-size:14pt;font-family:宋体;"> "font-size:14pt;font-family:宋体;">三、职业病危害评价收费实行**指导价（限价），从事评价工作的职业卫生技术服务机构应按照本通知收费标准收取费用。具体收费标准见附件。
"font-size:14pt;font-family:宋体;"> "font-size:14pt;font-family:宋体;">四、职业卫生技术服务机构与委托方应事先签订服务合同，明确双方的权利和义务。
"font-size:14pt;font-family:宋体;"> "font-size:14pt;font-family:宋体;">五、职业病危害评价收费使用税务票据，照章纳税。
"font-size:14pt;font-family:宋体;"> "font-size:14pt;font-family:宋体;">六、本通知收费标准试行一年，在执行中有何问题请及时报告我们。
